Parent-School Contract (Form 4)

This is a contract entered into by ACADEMIA DE SOPHIA INTERNATIONAL, a private, non-sectarian school duly recognized by the Department of Education, located at 3 South Drive, Baguio City, represented in this agreement by the Principal and Parent/Guardian;

WHEREAS, ACADEMIA DE SOPHIA INTERNATIONAL is committed at providing the best quality of education that the child deserves, where all activities, systems, and procedures within the school are founded on its philosophy, vision and mission, and motto;

WHEREAS, I/WE, as parent/s/guardian/s, have made a decision to enroll my/our child at ACADEMIA DE SOPHIA INTERNATIONAL aware of its philosophy and mission-vision in education, which emphasizes academic rigor, leadership and service, and internationalism;

WHEREAS, my/our decision to enroll my/our child at ACADEMIA DE SOPHIA INTERNATIONAL is upon my/our desire and will to be a partner in the education of my/our child;

NOW, THEREFORE, in consideration of the following, I/WE agree with the following:

1. I/We agree to send my/our child/ward to school every day on time and prepared for study except for reason of illness. I/We agree to bring my/our child/ward before 7:25 AM so as for him/her to attend classes.

2. For whatever problem or concern about my/our child/ward, or any situation, conflict and misunderstanding indirectly and directly related to my/our child/ward, I/we agree to immediately contact the teacher/adviser concerned or the Principal to personally discuss and settle such matter. I agree to NEVER DIRECTLY CONFRONT ANY CHILD WHO MAY HAVE CONFLICT WITH MINE.

3. Extra-curricular activities are part of the school program, and play a significant role in the integral development of the child. I/We therefore agree to let my/our child/ward actively attend and participate ALL activities and programs.

4. I/We agree to WORK CLOSELY WITH THE CLASSROOM TEACHERS IN ALL SITUATIONS whether at home or in school by attending programs, meetings and conferences, and activities to ensure that my/our child/ward will think and act with honesty, integrity, humility, and perseverance. I further recognize that my absence to a scheduled conference/general assembly is construed as my agreement to all matters discussed and resolved, without further complaint. I further agree to personally obtain the report card of my/our child/ward every quarter and to discuss with the adviser the progress of my/our child/ward.

5. I/We agree to provide a suitable place and time for homework and to review, sign, and return the Communication Notebook daily.

6. I/We agree to limit and manage the time that my/our child/ward spends with televisions, video games, and other noneducational activities during class days.

7. I/We agree to oversee my/our child/ward in using textbooks with proper care, and return the issued books at the end of the school year in the best of their condition. In case a book is destroyed or mishandled, I/we understand and agree to pay 150% of the original price of the book or to replace it with the same book.

8. I/We agree to ensure the transportation of my/our child/ward. I/We understand that it is my/our duty to inform the school of the school service of my/our child/ward. I/We also understand that on regular class days, without schedules or appointments, I/we are not allowed to stay in the school campus during class hours and beyond 5PM, I/we am/are to pick up my/our child/ward not later than 5:30 PM.

9. I/We understand the policy of the school requiring my/our child/ward to maintain a grade of at least C in all subjects. Academic warnings are issued each quarter to any student who incur three Cs, two Cs and one D, and one or more Ds. An academic probation may be issued after two academic warnings. If no improvement is noted, the student may be asked to withdraw from the school.

10. I/We agree to let my/our child/ward to attend remedial classes beyond regular class hours if my/our child/ward needs it. I/We agree to let my/our child/ward to stay in school as scheduled.

11. I/We agree that as parents, we acknowledge that I/we are the primary person/s responsible to shape and nurture the behavioral health of my/our child; ADSI supplements the shaping of character through its programs which the parents SHOULD SUPPORT AND COOPERATE WITH; and that, in the event that the child’s behavior in School has become excessive, is it the duty of the parents to support School efforts to guide the student’s conduct in School.

12. I/We understand that enrollment in ADSI is a privilege; therefore, I/we, as parent/s/guardian/s, agree to abide and follow the school’s rules, regulations, policies, and operating standards as stipulated in the Student and Parent Handbook. I/We understand that the school has the right and academic freedom to refuse readmission or to require the withdrawal of my/our child/ward from the School at any instance that I/we refuse or fail to abide and follow the rules and regulations of the School.

13. Finally, I/we understand and agree that not fulfilling these agreements may result in a request to appear before the Principal, and may necessitate me/us to withdraw from the School any time, and that I/we should withdraw from the School voluntarily if the School policies on student conduct and discipline are incongruent to their my/our own policies.
